Antidepressant Review Form

If you are due to have an antidepressant review, please read the following information and complete the below form. If your mood has been stable for 6 months or more you may be able to reduce or stop treatment with advice.

For some people at higher risk of relapse up to 2 years of treatment is advised.

For Anxiety a year of treatment is recommended.

Some people are better on treatment indefinitely.

  • If you have intrusive suicidal thoughts or plans speak to the duty doctor as an emergency
  • Please make a GP phone appointment if you wish to discuss reducing/stopping medication or discuss any other aspect of your care
  • If you simply wish to continue medication, please complete the form below and then request your medication through your usual way. i.e. through the prescription request page on this website or through pharmacy.
