Register with us as a New Patient

How to Register

We are currently accepting new registration applications. If you wish to register, please collect a form from the practice to take home and complete. When it is returned we will need to see everyone that is applying and over the age of 16 in person. You will also need to bring your ID and proof of address. If under 16 a Birth certificate will be accepted.

Please note – Forms can be collected from Tuesday mornings onwards and should be returned Tuesday to Thursday between 12pm and 5pm. Registration forms will not be handed out or taken in a Monday or Friday. This is due to the demand on these days.

Alternatively if you prefer you can download, then complete the forms, and present at reception with your identification, and proof of address. This is required for all patients registering with the Practice.

If you have been removed from a GP’s list, and cannot get another doctor to accept you, please telephone 0131 275 7038.

People who are homeless may access local GP services. Alternatively GP, nursing and mental health services are available at TAP (The Access Point), 17 Leith Street, Edinburgh and at the Cowgate Clinic, 20 Cowgate, Edinburgh. Advice regarding housing and social work services are also available at TAP (The Access Point).
