REGISTER WITH THE PRACTICE (INCL. NEW PATIENT MEDICAL) ?

Registration forms can be obtained from the reception desk. The receptionists can help you with any enquiries you may have. Please ask them if you need help to complete your form.

If you have just joined the practice, it may be a few months until we get your previous medical records. It is, however, very useful for us to know a bit about your medical history while we are waiting for your notes to arrive. To help with this you will be asked to have a new registration medical; this is usually performed by the nurse and mainly involves asking you questions about your past and present health.
